  • Open to all Carbon County High School Athletes
  • Required for 23-24 student eligibility
  • $25 Cash/Check at time of physical
  • 100% of fees donated back to your school’s athletic department
  • Parent/Guardian signed MHSA form required at registration and found at mhsa.org
  • Shorts and tee shirts recommended
